Teal'c claims that he witnessed Apophis' final breath in "Serpent's Song". However, he was in the Stargate Operations room at the moment of Apophis' death.

Goofs... 

When the SG teams are firing at the Jaffa behind the forcefield, the fluctuations where the bullets hit are clearly visible. The bullets strike at various points all across the force field, many of them nowhere near the target. Also at one point, one of the SG soldiers fires their automatic weapon at a 45 degree angle right in front of the camera. At that angle so close to the gate, the bullets would actually fly right over the gate, well over the heads of anyone in front of it.

General George Hammond is talking with Major Paul Davis about forming three new SG teams - SG-15, SG-16 and SG-17. But it takes over a year before the first of them (SG-15) was finally mentioned in "Divide and Conquer".

O'Neill's humorous words to Trofsky, "She's gone. She is no more. She's an ex-goddess. She's… well, let's face it, she's a former queen" is reminiscent of Monty Python's "Dead Parrot" sketch, in which an exasperated John Cleese tries to explain to the dull shop attendant that his parrot is dead.

Hathor's Jaffa came from many different Goa'uld (Serpent Guards from Apophis and Klorel, Horus Guards from Heru'ur, and others). She has obviously spent much of her time over the past year and a half gathering dissidents and defectors from Goa'uld all over the galaxy.

The title is presumably meant to invoke the 'out of the frying pan' imagery, as well as tying into the 'Out of Mind' precursor.
