And while Shaun seems to be thriving in Guatemala, Lea is still struggling with the loss of her baby.

Dr. Shaun Murphy must perform a risky surgery on a patient without electricity when the power suddenly goes out at the hospital in Guatemala. Additionally, Dr. Lim and Dr. Mateo Rendón Osma's relationship deepens as they overcome difficulties during their surgery.

Shaun and Lea's upcoming engagement party has everyone in a festive mood after their return from Guatemala. Meanwhile, a young single mother learns her son may have contracted his cancer from a surprising source, and Mateo finds out if his previous issues in America will be resolved.

Meanwhile, Dr. Glassman is apprehensive when a figure from his past reaches out for help.

Now that Shaun has called off the wedding, he is forced to confront Lea about their relationship and determine what he really means to her. Meanwhile, the team debate their opinions on plastic surgery and Dr. Audrey Lim hopes to expose Salen's cover-up over an infant's death to take her down before it is too late.

In an effort to please Salen, Andrews takes on a high-profile case and assigns Shaun and Allen to help perform a risky surgery to restore the voice of a famous pop star. In addition, Reznick and Park treat a man whose 17-year-old son, Cody, is confined to a wheelchair due to complications related to Kabuki Syndrome

Salen takes extreme measures when she learns that the surgical staff have joined Lim's efforts against her. Meanwhile, the team treats a liver transplant patient whose organ donor unwittingly gave him cancer. And elsewhere, Jordan, Asher and Andrews look for a surgical solution to remove a pregnant surrogate's cancer in such a way that she won't also need a hysterectomy to survive. 

Dr. Shaun Murphy makes a personal connection with a precocious young patient which threatens his objectivity. Meanwhile, while volunteering at a long-term care facility, Dr. Morgan Reznick realizes she could help Dr. Aaron Glassman's clinic by offering telemedicine services for nursing homes and care facilities.

Drs. Lim, Park and Wolke treat a woman with Valley fever and discover a secret she has been hiding from her fiancé. Meanwhile, Drs. Murphy, Allen and Andrews treat a 45-year-old virgin and help her make a big decision about her life.

While Drs. Lim, Murphy and Wolke help a teenage "biohacker" whose self- experiments begin to compromise his health, Drs. Morgan, Park, Jordan and Glassman treat a young woman who wants a controversial surgery to treat her depression and chronic pain. Additionally, Dr. Reznick looks to take the next step in her relationship with Dr. Park.

When someone mistakenly brings a hallucinogen-laced dish to the hospital's company potluck, much of the hospital staff and doctors are incapacitated -- leaving doctors Murphy, Glassman and Allen forced to take on all the hospital's cases, further exacerbating Dr. Murphy and Dr. Glassman's conflicting work styles.

While Doctor Andrews treats a foster kid for injuries sustained at his group home, Shaun and team look to Lea's expertise with cars and auto repair to help a patient whose iron lung has broken. Meanwhile, the patient's niece, a documentarian, takes an interest in Shaun.

As Shaun and Lea head toward their wedding day, a documentary film maker has chosen to follow their lives to showcase relationships and weddings of atypical couples.

Sophie takes Lea out for a day full of luxurious wedding planning. And after a near fatal oversight, Dr. Lim senses an underlying issue is behind nurse Villanueva's unprofessional behavior. 

Shaun and Lea make a big life decision and are pleasantly surprised by the support they receive from their friends at the hospital. Meanwhile, when Dr. Wolke's parents show up with a revelation, Asher struggles to accept that he and his father may never truly understand one another on the season finale.

Shaun and Lea's long-awaited wedding reception is interrupted by a violent attack at the hospital and the entire team must mobilize to save the victims. Meanwhile, the trauma of seeing people he cares for in life-threatening situations is affecting Shaun in more ways than one.

Shaun and Lea soon learn that their surprise pregnancy may also come with additional complications. Meanwhile, Doctors Park, Reznick and Allen treat a teen with Gardner's syndrome whose past surgical history jeopardizes the outcome of his current one. 

On the way to their babymoon, Lea and Shaun get a quick lesson in parenting when they rush to aid an injured dog. Meanwhile, Dr. Danica Powell secretly performs an operation on a friend that could jeopardize her career.

Shaun invites Aaron to stay with him while he has exterminators at his house and Lea realizes both men have a lot more in common than she realized. Meanwhile, Dr. Morgan Reznick wrestles with a big decision about her career and her personal life.

Shaun and Lea clash over their parenting styles and worry about what will happen to their relationship when their child is born; Dr. Lim must find a way to save the damaged lungs that one of her long-time patients has been waiting for.

When a 3-year-old arrives at the emergency room, the team immediately recognizes he is having a stroke; Dr. Shaun Murphy's diagnosis of the underlying cause doesn't solve the problem, which has Dr. Park searching deeper.

Dr. Jared Kalu makes a surprise return to San Jose's St. Bonaventure Hospital with his billionaire patient; Park must treat the man his wife had an affair with and try to find a way to forgive him. 

Dr. Shaun Murphy seeks legal representation to help him win a case and puts his faith in a promising, young lawyer who has obsessive compulsive disorder. 

Dr. Shaun Murphy must learn how to work with Dr. Jared Kalu again and remind him that he must start all over at St. Bonaventure; Dr. Murphy treats a teenager suffering from neurological problems with a very high-risk procedure.

When a patient comes in with persistent and lingering COVID symptoms, the team discovers she may be dealing with something harder to treat; Andrews and Villanueva must contend with their hierarchy at the hospital and its effect on their relationship.

Drs. Andrews and Lim clash over a patient split nearly in half, which also reveals her issues with Dr. Kalu's return; Dr. Glassman gives Dr. Murphy some fatherly advice as they await news about Glassman's recent tests.

Dr. Marcus Andrews introduces some of the team to a patient whose optimism and faith seems to cause Dr. Asher Wolke the most skepticism in light of his own relationship with religion. 

Dr. Glassman and Dr. Murphy's relationship may be irreparably damaged following a tense moment during surgery. While Dr. Reznick struggles during her parental leave, Dr. Park may just be the one she needs the most.

Shaun and Lea are heading to the delivery room and everyone from the hospital is also there, except for one very important person. Meanwhile, a tragic accident involving Drs. Perez and Kalu pulls everyone away, including Dr. Murphy.

Shaun and Lea adjust to parenthood as they debate the importance of schedule and routine for Steve. Meanwhile, Shaun takes on his first case back at the hospital with two baby patients in need of the same heart.

Shaun struggles to accommodate the newest member of his surgical team, Charlie, who interferes in a patient's relationship with his daughter; Park tackles a tricky brain tumor; Lea and Morgan adjust to motherhood.

Shaun continues to be tested by Charlie when their newest case prompts her incessant stream of questions – all in pursuit of an answer. Meanwhile, Morgan struggles on who to name as guardian for Baby Eden should anything happen to her.

Shaun and Lea struggle to balance their parental duties with their personal lives. Meanwhile, Park and Morgan try to find the time and space to reignite their romance amidst the demands of parenthood and work, and Lim's mother makes a surprise visit.

Asher's views on the construct of marriage complicate his future with Jerome. Meanwhile, Asher also briefly revisits his religious past to aid a patient's conversion to Judaism for his fiancée.

The team must deal with a mass casualty event that forces them to put aside their emotions following a recent tragedy.

Shaun and Jordan's patient is in dire need of a kidney transplant. When they find the perfect donor, they also discover that he believes he is Jesus which could compromise his ability to give consent to the surgery.

Shaun, Jared and Charlie's young patient need his parents to agree on an important medical decision that could greatly improve his health; however, his parents are unable to agree on the correct path which spurs the team to try and intervene on behalf of their patient.
