Gleb Kaverin, an experienced surgeon. On vacation, he sails along the Volga on a small ship when the unexpected happens on board. One of the passengers of the ship, a pregnant woman, suddenly felt very ill. Unable to call for medical help, Gleb remembered his first specialty - obstetrician-gynecologist. He was able to save the mother and the unborn child. Gleb is shocked by his wife's betrayal. Had his Marina cuckolded him? On nerves, Gleb breaks the arm of Berdyaev, an impolite client of the clinic.

After Gleb Kaverin beat Berdyaev in the clinic, he found himself in an unpleasant situation. The police detained him for fighting and resisting arrest, and now he faces a criminal case. The head of the ROVD, Lieutenant Colonel Startseva, who is well acquainted with Kaverin, agrees to help him, but on condition that, first, he reconciles with Berdyaev. However, she has one more necessary condition. Gleb understands that it will be difficult for him to cope with the situation without the help of the police. He decides to accept Startseva's offer and agrees to all her conditions.

Startseva's second condition in exchange for help with Berdyaev is that Gleb Kaverin should help her do paternity tests for her child. The woman is not quite sure that she is expecting a child from her husband. He will have to get a job as an obstetrician at the perinatal clinic again, and there he will become a subordinate of his former classmate Sorokin. Kaverin starts his new job and is faced with difficult medical cases that have to be solved together with his new team.

But, as luck would have it, it turned out that Yelagina was Sorokin's ex, Kaverin's enemy and rival since the days of the medical institute. Once they fought for Marina's heart and she chose Gleb. During a difficult birth, Kaverin has to calm the violent young father with his own methods. But he turned out to be the son of the local prosecutor. He started threatening Kaverin with dismissal.

Elagina found herself in a terrible situation. The husband of her former patient, lawyer Leonid Agalidi, discovered that the daughter conceived with IVF was not his. Now he is suing the clinic for a tidy sum, which puts it at risk of closure. However, thanks to Gleb Kaverin, Yelagina got out of this situation. Gleb proved that this was not a medical error and Leonid's accusations were unfounded. Nevertheless, this harsh lesson reminded all clinic staff of the importance of following protocols and procedures, including during IVF procedures, in order to avoid similar troubles in the future.

For example, he is struggling with the consequences of a traumatic past, he witnessed his father's murder as a child. Gleb seeks help from a friend, a psychotherapist, to cope with his emotional difficulties.

Artyom Stetsenko, the head of the local health department, arrives with his young wife at the medical center to be examined. There it turns out that the woman lost the fetus. Stetsenko begs Kaverin and Sorokin to help him, to go to replace the child. Marina comes to Gleb with the stunning news that she is pregnant. It becomes clear that everything is over between her and Gleb. Now Kaverin can no longer be bothered to be with Yelagina, who has always supported him in difficult moments.

Bragin demands that Kaverin and Zharova organize the escape from custody of a pregnant prisoner, who should soon be transferred to their center. The prisoner arrives with a stomach wound, which turns out to be part of an escape plan. Kaverin is forced to participate in the staging in order to save the woman's life. Sorokin, sensing something is amiss, tries to intervene, but he is neutralized. During the escape, Gromova goes into premature labor...
