I would say a new show and connect it with a relation to each other.
The air time for Cosmos S3 episode 13 appears wrong (8pm eastern). I added a duplicate with the same name as a special (9pm eastern). Someone want to fix?

The episode was one of two episodes scheduled to air last year to end the season (as seen here: http://www.thefutoncritic.com/showatch/american-dad/listings/), but was held back and even though S16E01 aired last week, they've continued to label these two heldback episodes as being part of the previous season.
The other held episode is set to air next week, which would become S15E22 here.

Season
It's not allowed for two seasons to run simultaneously. For example, there cannot be an episode in season 2 with an airdate that's before the last aired episode in season 1. If a source provides season numbering like this, it should be discarded and you should look at the next best source instead.
Episode number
The unique sequence number of the episode within its season. This should always reflect the order in which episodes were originally premiered.
This means all new episodes aired after ep. 16x01 'till the end of the season are belongs to season 16, and should be numbered sequentially according to broadcasting order.

Is it worth changing this guide to year-based seasons rather then having a bunch of specials listed under the episodes the BBC deemed at the time "regular" episodes. There is nothing different from the newer "special" episodes to those that aired regularly, the BBC are listing any that aired on BBC Three (online channel) without a season number, which is why I think we've started listing them as specials. There has now been more episodes released online only then there was when BBC Three was a linear channel.
Also the newest episode that aired on Sunday night actually aired on BBC Two first before been added to iPlayer so didn't even premiere as a BBC Three show (I didn't want to add the new episode before making any other changes that may need to be made). I'm assuming any new episodes moving forward will also air on BBC Two first, but there hasn't been any "official" announcements about this.
We do list Louis Theroux documentaries in year-based order (very similar in type of production and also airing sporadic new episodes on BBC Two)

I won't recommend doing that.
Firstly, there's a season separation at the show's website. Some of them have numbers, like series 2 and series 6, the rest have names. And some years contain more that one series (2013 and 2015), so it won't be really correct to mix them together.

No problems :). However the show hasn't aired in seasons for 5 years now, and the official website appears to be missing some episodes altogether as well.
Does the network need changing to BBC Two in light of the newest episode airing on their on Sunday, I'm aware of the lifetime network taking precedence, however with it been in the same family of channels I wasn't sure if this still applied. Also the episodes are made sporadically so I don't know if this was just a one-off or if the new episodes will be airing on BBC Two for good now or not.
