Paige is coordinating a fund raising party for her father's Senate campaign. She goes inside to talk with her mother Ellen who is sitting alone in her bedroom. Paige tries to engage her in conversation about the party but her mother is clearly depressed about something. When Paige turns her back, Ellen disappears from her room.
Evan is baking cookies and cream puffs for Paige's party when Hank joins him. Evan is in a much better mood than when the previous episode ended and Hank asks him about it. Evan tells him he misunderstood Paige's absence and that she was preparing for the party, not keeping company with the art dealer. He rationalizes her father's not knowing her whereabouts as being due to an overwhelming campaign schedule.
Paige goes looking for Ellen and sees her outside walking in a daze. Paige rushes after her and finds her where she has fallen off stairs onto the beach. She calls Hank and he answers, agreeing to come right away without giving away her identity to an eavesdropping Evan, instructing her to call 911.
Hank examines Ellen and stabilizes her neck with a brace. When he asks where the paramedics are, Paige tearfully admits she did not call them, not wanting them involved. Hank finds only cuts and possibly a broken bone in the foot but wants x-rays done just to make sure. Paige seems reluctant so Hank asks her to get a blanket so he can speak with Ellen alone. She admits not being happy for a long time and Hank suggests she talk with someone.
Hank nexts shows up for a consultation with a new patient and meets Divya. He tells her about his early morning patient and she asks if there is anything she can do to help. He asks to be filled in on the new patient, Simon Field, a six year old with asthma. Simon is being cared for by his nanny Toby Thompson while both parents are out of the country. Simon also seems to be afraid of most everything so Hank must listen to Toby's lungs before Simon lets himself be examined. Hank brought a different type of nebulizer for Simon, one with a mask instead of a mouthpiece and Toby at first balks, but then decides Simon needs to try new things.
At Hamptons Heritage Hospital, Paige is with her mother in her room where she was admitted. Hank stops by to check on her, having already learned that the x-rays revealed no internal injuries. When Hank asks about the Psych exam, Paige says they will come back after she awakens, but Paige is afraid she will check herself out of the hospital before they can speak with her.
Hank pushes for more information and Paige reveals Ellen has been depressed since before she was born. The last time she saw a doctor for treatment was over eight months ago, and she saw a military doctor friend of her husband's for his discretion, in order not to affect her husband's campaign. Paige admits this is what she started to ask Hank for help earlier in the summer, and why she couldn't tell anyone, because it was not her secret to reveal.
Toby shows Simon the new nebulizer and tries to convince him to put the mask on, but he turns the tables and insists she try it first.
Paige goes home to pack a bag for her mother when Evan arrives. He has brought the baked goods and is surprised to see her there, thinking she was away on a trip for the campaign. He asks her if she ever left town and when she says no, he accuses her of lying to him, and not for the first time. She tries to explain but he keeps interrupting, and asks if she is seeing Kasabian the art dealer. This really surprises Paige and she gets angry, telling him that they are through.
Hank follows up with Simon where he is speaking to his parents over a Skype connection to Beijing. While he walks off, Toby reveals she hurt her head when she fell over one of Simon's toys. His examination reveals she needs stitches and Hank quickly sews her wound while Simon is distracted.
At home, Evan busies himself destroying the cream puffs with a golf club when Paige's father, the General arrives. The General is furious with Hank and tells Evan when Hank returns to tell his brother to stay away from her and off his property. When Evan asks what is going on, he finds out that Paige called Hank the day before and Hank treated Ellen without telling Evan.
At the hospital, Jill stops by to speak with Divya about her upcoming date with Dr. Paul Van Dyke. Divya says she is going to cancel the date but Jill talks her out of it, saying she needs to jump into the dating pool.
Hank arrives home to an angry Evan who starts yelling at him. Evan accuses Hank of keeping secrets about Paige from him and when Hank claims doctor-patient confidentiality, Evan calls bull, saying family is more important. He even takes credit for Hank's current success, saying Hank would still be sitting in his darkened apartment without Evan pushing him. He then takes a golf club to Hank's car windshield and blames Hank for his breakup instead of his own insecurities and lack of trust.
Later, Hank tells Divya the story and she tries to console him, saying Evan will eventually forgive him. Hank says he knew better, that secrets always come back to bite you, which strikes home with Divya who is keeping her second job at Hamptons Heritage a secret from both Hank and Evan.
After Simon's asthma treatment, Toby tries to get him to get into the swimming pool. As he usually does, he insists she go first, taking her by surprise by pushing her into the pool. For some reason, she is unconscious, floating face down in the pool and Simon thinks she is playing. When he realizes something is really wrong, he gather his courage and jumps in and manages to awaken her and get her to the side of the pool and they call Hank.
Hank examines her and finds nothing obvious but wants to do more tests to rule out any neurological problems. He worries her earlier injury might have been caused by fainting, instead of tripping over a toy. Because of that he cannot leave her alone with Simon. She says the housekeeper is coming from the city later that evening and Hank agrees to stay until she arrives, and will even have a friend bring over some pizza.
Paul takes Divya on their date on the back of his motorcycle, and she is not happy he didn't warn her so she could dress properly. They walk toward the restaurant but he makes a side trip to a dance club to her disgust, saying he wants to work up an appetite.
Evan arrives at the Collins house where he and Paige make up immediately, promising to never keep secrets.
Hank and Jill are playing outside with Simon and Toby after the pizza party. Soon Toby takes Simon for his asthma treatment and bed and leaves Hank and Jill alone where Hank apologizes for changing their date night. When Hank and Jill try to enter the house to see if the housekeeper has arrived, they find the doors locked. Hank spies Toby through the window and gets her attention. As she walks toward the door she collapses on the floor and convulses.
Hank breaks the door glass to gain entry while Jill retrieves his medical bag and portable heart monitor. While Jill performs CPR, Hank connects the monitor to reveal Toby's heart is fibrillating. He uses the electric paddles to shock her heart to no avail. Hank realizes she is experiencing Torsades, which is why the defibrillator didn't work. Hank injects her with magnesium and she comes around quickly. Hank insists she needs to go to the hospital and Jill agrees to stay with Simon.
Despite her misgivings, Divya is having fun with Paul, even dancing with him. The dance club has a surf board ride, similar to a bull ride where you try to stay on board while it twists and turns. Paul slips the operator some cash and then pretends to be thrown from the board, and feigns unconsciousness. When Divya becomes alarmed and almost starts mouth to mouth resuscitation, she is furious when she discovers he is faking. He apologizes and asks for a second chance, saying it has been a long time since he dated and he is out of practice.
Hank checks in on Toby at the hospital and tells her it is confirmed she has a condition where her heart can jump into an arrhythmia when she is startled. Hank is not sure what started triggering her episodes until Simon stops by, and he finds out Toby tried the nebulizer first, as she usually does to calm his fears. Hank realizes the albuterol of the asthma medication is the cause of her recent episodes.
Divya and Paul are having dinner on the back of a yacht anchored in the marina, having missed their dinner reservations at the restaurant. Divya tells Paul the story of her failed wedding and disinheritance and he tells her the details of his normal boring life. He suggests they will have better luck on their next date and Divya says there won't be one. Paul then suggests they go out as just platonic friends getting to know one another, since they both need the dating practice.
Paige and Evan bring Ellen her dinner to her room where she is reluctant to join the fund raiser party. Ellen seems in better spirits and bids the couple go have fun, saying she will be fine. They arrive at the party where one of Ellen's friends asks how she is doing, the rumor mill having started quickly. Paige hesitates and Evan jumps in telling everyone Ellen just has the Summer flu and is resting per doctor's orders. Paige watches Evan with gratitude and affection for helping protect her mother.
Hank arrives at the Collins home to check on Ellen and Evan meets him at the door. Evan tells Hank he is not going to apologize and Hank says okay, then says he is sorry for his part in the complicated situation. He hands Evan two bottles of pills that Ellen left at the hospital and starts to tell Evan that the psychiatrist can see Ellen as an outpatient when the General arrives, telling Hank to leave. The General then asks Evan if he is staying or going, basically asking him to take sides. Evan closes the door on Hank, having made his choice.
