HankMed is having their morning staff meeting without a few key members. Dr. Sacani is away at a conference and as Hank wonders where Dr. Paul Van Dyke is, he stumbles in, obviously drunk. Evan volunteers to sober him up while Paige starts rescheduling his patients with the exception of one patient that Divya will cover. Hank and Divya leave to see Divya's friend Tom Fancy, a veterinarian specializing in large animals. Tom insists his problems are just stress related, but Hank insists on running lab tests to make sure.
Later Hank stops by Boris' estate to check on Dmitry and to perform another phlebotomy, but Dmitry is in the middle of business with Boris and is in no mood to cooperate. Hank chides him about not taking his Insulin before his meal and checks Dmitry's blood sugar. After Dmitry takes his Insulin shot, they arrange to meet in an hour for the phlebotomy.
Hank returns in an hour and installs an Insulin pump to Dmitry's abdomen. The pump will automatically inject low doses of Insulin and can give a boost amount depending on the results of a blood sugar test.
Paul wakes up and wonders why he is at HankMed. He also has a contusion above his eye that he got from his buddy the previous night when they were out drinking. Evan calls Divya to come back and stitch the cut. Paige is upset that Paul refuses to apologize for his unprofessionalism.
Hank is called back to Boris' estate to check on Dmitry who has become ill. Dmitry has been eating pastries and not checking his blood sugar level like Hank instructed him and is close to a diabetic coma. Hank scolds him but Dmitry tells him he has been diabetic for years and knows how to handle it. Hank tells him unless he wants an implanted glucose monitor in addition to his Insulin pump, he better start taking this seriously and suggests he hire someone to stay with him 24/7 to make sure he keeps up with his diabetes. Dmitry says okay, and says Hank is hired for the job.
As Hank and Boris leave, Hank tells him he doesn't want to work for Dmitry and can't think of anything else to do for him. Boris says he needs Dmitry ready for the summit the next day and Hank agrees to keep thinking of a solution.
Evan demands an explanation from Paul for his condition. When Paul refuses to explain, Evan fires him.
Hank and Divya return to see Tom and decide to do an x-ray of his intestines to find out what is causing his symptoms. The x-rays don't help and the next step is an endoscopy which Tom is reluctant to do since he is so busy with work. Divya tries to reason with him but Tom is working 24/7 doing his job and going through a bitter divorce.
Hank stops to Boris who seems curiously interested about his relationship with Christina, the translator working for him. He even goes so far as to offer his car and driver to take them to dinner after the summit. Hank tells him he has no further ideas to help Dmitry other than to check his blood sugar hourly during the conference, and no long term solutions. Hank is interrupted by a call from Divya who is concerned about Tom, who she cannot contact via phone.
Hank and Divya rush to the farm where Tom is treating several cows. Tom is apparently dizzy, and when he moves behind a cow gets kicked in the face, losing a tooth in the process. They treat him and try to get him to go to the hospital but he insists he has a patient that needs him and he won't leave the cow. Hank offers to treat the cow himself if it will get Tom to the hospital.
Evan and Paige go for lunch at the bar where Paul said he was drinking the night before. Paige figures out what Evan is trying to do and tells him he should not second guess his decisions, but Evan asks the bartender about Paul anyway. When the bartender relates the story, Paige is touched that Paul is a hopeless romantic mourning the loss of his girl and tells Evan he needs to rehire Paul.
Hank and Divya are driving Tom to the hospital but get stuck in traffic. A comment Tom makes about guide animals gives Hank an idea to help Dmitry. As he talks with Tom he notices Tom slurring his words and recognizes a stroke in progress. Hank improvises again and commandeers an ice cream truck to take Tom to the hospital, using ice cream bars to keep Tom's core temperature down during the trip.
Hank visits Dmitry and brings a dog with him that has been trained to detect the scent of low or high blood glucose levels. Dmitry is skeptical and demands proof. Hank has him overindulge in rich foods and the dog detects the change in his blood sugar. Dmitry is impressed and thank Hank for the dog but Hank says that the dog is just a loaner from his veterinarian friend, but Dmitry pays no attention. Boris and Dmitry enter the conference and before the doors close, Hank sees some of the attendees including Arab sheikhs and men in military uniforms.
Evan and Paige go to see Paul where the truth comes out. He is not heartsick over a girl, but over his pet rabbit. Evan and Paige can't help but laugh at him which is why he never told them the truth in the first place. The trip to the bar was for liquid courage to enable him to put the rabbit down. When Paul takes them to meet his pet, Evan discovers that it has already died.
HankMed holds a memorial for the rabbit and begin to cast its ashes into the ocean when a lifeguard notices them and sends a police officer to investigate. HankMed scatters.
After the conference, Hank meets Christina for a private drink where they kiss as the episode ends.
